It happens for us too randomly. All we do is re-try or re-run the failed process.

aparently there is some dependency in your rule that differs per situation. But with such a generic question its hard to give any advice at all. Perhaps you can give some more info like the code of your confirmation rule or a description of situations where it does and/ or doesn't work.
usually with troubleshooting step 1 is to narrow it down as much as possible so try to eliminate dependencies whereever possible.
